An individual who has been treated by the Ambulance Service has the right to request their data from the service. This is done via a Patient Data Request which can be completed here.

When completing data requests you will need to provide:

• The name requesting person.

• The time, date and location of the incident.

• The PRF number (if known)

• A place to send the information for example a discord or email.

• Physical ID (You will be contacted with a date, time and station to go to to present this)

Note that we will not be able to provide data from requests that do not provide all of the above. The requests form is checked weekly on a Saturday so may be responded to slowly.
